      摘要 目的:通过分析特征性的声学参数,区分有效咳嗽与清嗓,并探究其与气流速度(峰流速)之间的相关性,为肺康复的咳嗽训练提供客观便捷的评估工具。 方法:收录60例健康青年志愿者仰卧位下的10次咳嗽音和10次清嗓音,对两种动作的6个特征性声学参数进行分析和比较。进一步选取11例受试者在仰卧位下进行5次咳嗽和5次清嗓动作,同步采集声音和气流进行相关性分析。 结果:咳嗽音的峰值幅度、峰值功率、峰值频率和聚合熵均显著高于清嗓音(P<0.05),咳嗽音的达峰时长占比显著低于清嗓音(P<0.01)。咳嗽峰流速分别与咳嗽音的峰值幅度(r=0.6,P<0.01)和峰值功率(r=0.55,P<0.01)呈显著正相关关系。清嗓峰流速仅与清嗓音峰值幅度呈显著正相关性(r=0.37,P<0.01),与峰值功率无相关性(r=0.0037,P=0.98)。 结论:声学分析可用于量化区分健康青年人的咳嗽和清嗓;咳嗽音的声学参数与咳嗽峰流速具有良好正相关性。咳嗽音的声学分析可为临床咳嗽训练提供便捷的评估工具。

Abstract:
      Abstract Objective:To distinguish effective cough from throat-clear by acoustic analysis and correlation analysis between sounds and airflow for providing an objective and convenient evaluation tool for cough training in pulmonary rehabilitation. Method:Ten episodes of cough and ten episodes of throat-clear sounds of 60 healthy young volunteers were recorded from in the supine position. Six characteristic acoustic parameters in the cough and throat-clear were analyzed and compared. Then,eleven subjects were further selected to perform five episodes of cough and five episodes of throat-clear in the supine position, the sound and airflow were collected synchronously for correlation analysis. Result:The peak amplitude, peak power, peak frequency and aggregation entropy of cough sound were significantly higher than those of throat-clear (P<0.05), and the peak time in cough sound was significantly lower than that in throat-clear (P<0.01). The peak flow of cough was significantly positive correlation with peak amplitude (r=0.6, P<0.01) and peak power (r=0.55, P<0.01) in cough respectively. The peak flow of throat-clear was significantly positive correlation with peak amplitude (r=0.37, P<0.01), but not with peak power(r=0.0037, P=0.98). Conclusion:Cough and throat-clear in healthy young people can be quantitativelydistinguished by acoustic analysis.The acoustic parameters of cough sound are positively correlated with the cough peak flow. The acoustic analysis of cough sounds can provide objective and convenient evaluation tool for clinical cough training.
